What We Do
We provide consultations to help you have a smooth transition from admission to discharge. Electronic medical records will be transmitted securely between the hospital and skilled nursing facility to make the transition as seamless as possible. We will also consult with hospital physicians and discharge planners to ensure that your admission to the skilled nursing facility is the best option for you. If you need SNF care, we can provide certification.
Upon admission, our clinical team will create a tailored care plan, which will be strictly monitored and modified as necessary. Our goal is to help improve your strength and enhance your quality of life.
